Abstract
Efectividad de la calidad de los registros clínicos obstétricos en el diagnóstico presuntivo, Hospital II-2 Tarapoto - 2023
El desafío mundial en los registros clínicos de Obstetricia llevó al Ministerio de Salud a implementar medidas legales para garantizar servicios eficaces y respetuosos con los derechos de las usuarias. Aunque los registros deben ser transparentes y concisos, se observa incumplimiento por parte de los profesionales de salud en su correcta cumplimentación. El estudio tuvo como objetivo determinar la efectividad de la calidad de los registros clínicos obstétricos en el diagnóstico presuntivo según la NTS N° 029, Hospital II-2 Tarapoto - 2023. El estudio fue básico, diseño de la investigación es no experimental, cuantitativa, correlacional de corte transversal. La muestra estuvo integrada por 467 registros clínicos obstétricos de las pacientes atendidas en el servicio de consulta externa de enero - junio, 2023. Resultados: La evaluación de los registros clínicos obstétricos del Hospital II-2 Tarapoto evidencia alta calidad en varias dimensiones, con fortalezas en la historia clínica, pero debilidades en el diagnóstico presuntivo III y errores frecuentes como legibilidad deficiente (57.6 %), uso incorrecto del CIE-10 (23.6 %) e inconsistencia diagnóstica (18.8 %). Aunque los diagnósticos presuntivos I y II muestran buena precisión, persisten discrepancias en patologías fetales y anemia. Se confirma una relación significativa entre calidad del registro y precisión diagnóstica (Rho = 0.449; p = 0.000). Se concluye que, la calidad de los registros clínicos obstétricos del Hospital II-2 Tarapoto presenta un alto nivel de cumplimiento en la mayoría de sus dimensiones, destacando especialmente la anamnesis, el examen clínico, el plan de trabajo y los procedimientos del obstetra con un 100 % en nivel alto.

Although records must be transparent and concise, health professionals fail to comply with their correct completion. The study aimed to determine the effectiveness of the quality of obstetric clinical records in presumptive diagnosis according to NTS No. 029, Hospital II-2 Tarapoto - 2023. The study was basic, with a non-experimental, quantitative, correlational, and cross-sectional research design. The sample consisted of 467 obstetric clinical records of patients atended in the outpatient department from January to June 2023. Results: The evaluation of the obstetric clinical records of Hospital II-2 Tarapoto shows high quality in several dimensions, with strengths in the clinical history, but weaknesses in presumptive diagnosis III and frequent errors such as poor readability (57.6 %), incorrect use of the ICD-10 (23.6 %), and diagnostic inconsistency (18.8 %). Although presumptive diagnoses I and II show good accuracy, discrepancies persist in fetal pathologies and anemia. A significant relationship between record quality and diagnostic accuracy is confirmed (Rho = 0.449; p = 0.000). It is concluded that the quality of the obstetric clinical records at Hospital II-2 Tarapoto presents a high level of compliance in most of its dimensions, especially highlighting the anamnesis, clinical examination, work plan and obstetrician procedures with 100 % at a high level.
